Why InMeD & LeMeDaRT?
Good perioperative care begins long before surgery.
Many relevant risks and influencing factors are already present before an operation. At the same time, important information is often generated at different locations, at different points in time, and within separate systems — and may become fragmented across the perioperative pathway or only become available late.
LeMeDaRT (Lean Medical Data at the Right Time) created the technological and clinical foundations for digital prehabilitation, multi-risk assessment and the active involvement of patients. InMeD (Intelligent Medical Data) extends the approach into a cross-sector care model that connects outpatient care, hospital, rehabilitation and home aftercare.
Both programmes consider the entire patient journey – from preoperative assessment and preparation through hospital care to postoperative recovery. The aim is to provide the right information at the right time for clinical decisions.
The key is not the digitalization of individual steps, but their meaningful integration into a continuous, data-based perioperative care pathway — rather than a sequence of isolated treatment episodes.
How can individual stages of care become one connected patient journey?
Multi-Risk & Individual Needs
How can preoperative clinical, cardiopulmonary, functional, cognitive, nutritional and psychosocial information in a digital multi-risk assessment make individual risks and support needs visible at an early stage?
Prehabilitation & Preparation
How can targeted preoperative interventions — physical training, nutritional optimisation, psychosocial support and extended pathways with cognitive training — prepare patients for surgery and recovery?
Cross-Sector Patient Journey
How can information and processes be meaningfully connected across general practitioner, specialist, hospital, rehabilitation, nursing care and home aftercare?
Patient-Centered Outcomes
How do digital accompaniment, prehabilitation and connected models of care affect recovery, function, quality of life, treatment adherence, PROMs, PREMs and other clinical outcomes?
